Юрий Михайлович работает нейрохирургом в городской больнице. Его день состоит из операций, осмотров больных, попыток выбить квоты на размещение пациентов. Каждый день он оглашает людям страшные диагнозы и, словно мольбы о помиловании, выслушивает вопросы о том, есть ли хоть какая-нибудь надежда. Его преследует один страшный образ – человека, обреченного на неподвижную и бессмысленную жизнь в состоянии «овоща». Этих несчастных людей он тоже вынужден наблюдать едва ли не каждый день. Выглядит так, будто он спасает им жизнь, но он понимает, что лишь растягивает во времени их умирание. Ему невыносимо думать, что такое может случиться и с ним самим. И он говорит своей подруге: «Если со мной что-то случится, убей меня! Просто введи по вене инсулин 400 единиц и все! Если любишь – убей!»
